First, it's usually a temporary failure. Check back, and it should correct itself. If it doesn't, then something more serious is wrong, and a quick fix wasn't available.

It usually occurs when there has been a hardware failure, or while were are in the middle of recovering from one.

Ensuring the safety of the data occasionally causes it to be made unavailable.
